Registration Form - Rhode Island Futsal League 2024-25

I understand that in addition to the league fees, each player must register with American Futsal Association (AFA).  The $15.00 per player fee and player releases are completed and paid online by the player/parent/guardian (P/P/G).  Players will not be eligible to participate unless payment is received and released authorized.  Outdoor player registrations issued from US Club, AYSO, USYSA, State Associations. etc. are not valid for registration with AFA.

I understand that in addition to the league fees, each coach and team manager must register with American Futsal Association (AFA).  The $20.00 per coach and team manager fee and releases are completed and paid online by each coach and team manager. Coaches and Managers will not be eligible to participate unless payment is received and released authorized.  Outdoor player registrations issued from US Club, AYSO, USYSA, State Associations. etc. are not valid for registration with AFA.

League refund policy: A partial refund will be issued when you cancel your registration before the registration deadline and receive a refund of your fees minus a $250.00 USD administrative fee per team. After the registration deadline, no refunds will be given.
